JOB SUMMARY: The Clinical Navigator position has responsibility for 2 different areas: neuro-oncology and breast health. On the neuro-oncology track, the Clinical Navigator will provide daily care coordination, case management, coaching, consultation, and intervention to neuro-oncology patients undergoing radiosurgery, radiation therapy, traditional surgery and chemotherapy. The specific purpose of the Clinical Navigator is: 1) to expedite and facilitate access to initial and ongoing care both in the Health System and the community from diagnosis to survivorship, 2) to personalize the care patients receive, providing an easily accessible and consistent gateway for patients and clarifying communication from multiple providers, 3) to partner with providers in developing an individualized treatment plan and educating the patient and family about it, and 4) to serve as a subject matter expert, collaborating to develop and refine clinical treatment pathways for the neuro-oncology and Sterotactic RadioSurgery program. The Clinical Navigator is patient-oriented, not physician or practice/division specific, and will facilitate care of the patient and family across the many disciplines and professionals required for their care. On the Breast Health track, the Clinical Navigator will work closely with the mammography department to provide care coordination, case management, education, consultation, and referrals to patients with diagnostic mammo patients in need of further diagnostic procedures, biopsy, genetic testing, and education. The Clinical Navigator will manage the High Risk for Breast Cancer Assessment Program to ensure completion of family history questionnaires, analysis of data, patient education, and referrals to the High Risk Breast Clinic and/or Genetic Counselor.

EDUCATION/EXPERIENCE:

  • Bachelors Nursing or Radiation Oncology, or ARRT certification/Associates degree required. If Bachelor's degree in Radiation Oncology, must complete a Certified Radiation Oncology Technology Program. If Bachelor's degree in Nursing, must be from an accredited school of professional nursing.
  • 2 Years Clinical Oncology Patient Care as Registered Nurse or Radiation Therapist or Mammography Technologist Required
  • 1 Year Case Management or Navigation experience Preferred

LICENSURE, CERTIFICATION, and/or REGISTRATION:

  • If RN: Registered Nurse with current North Carolina license -North Carolina Board of Nursing Required.
  • If Radiation Therapist or Mammographer: Radiography and Radiation Therapy (ARRT) Certification
  • American Registry of Radiologic Technologists Required. Healthcare Provider BLS/CPR required.

One of the following certifications is required within two (2) years from date of hire:

  • Oncology Patient Navigator
  • Oncology Nurse Navigator
  • Oncology Certified Nurse
  • Certified Breast Patient Navigator

Note: Must be certified by the Academy of Oncology Nurse & Patient Navigators or Oncology Nursing Society or National

  • Consortium of Breast Centers.
  • Must possess a valid NC driver’s license and a vehicle to drive.

ESSENTIAL FUNCTIONS:

  • Patient Identification, Introduction, and Intake. Works with appropriate physician providers, Tumor Registry, and Pathology Department to identify patients in the assigned diagnostic group who meet criteria for navigation services. Initiate introductory visit following timeliness guidelines per the navigation pathway. Complete intake interview, working to build a relationship, providing education and resources, and facilitating answers to questions or immediate assistance in removing barriers to care.
  • Facilitate Patient Referrals . Reviews patient chart and discusses with providers to identify needs for additional diagnostic follow-up, provider consultation appointments, or Oncology Care Team referrals. Assures pre-operative testing that is appropriate for patient needs. Coordinates care of patient between radiation oncologist, surgeon, and medical oncologist. Coordinates scheduling of tests: labs, x-rays; coordinates consults and collaborates with patient to schedule and explain all needed appointments.
  • Documentation. On the day of each encounter, fully documents assessments, referrals, patient progress, and known barriers to care in the appropriate section of the Electronic Medical Record.
  • Education. Educates patients and families on their diagnosis, prognosis, treatment status, and symptom management. Actively participates in outreach/community education and/or early detection/screening events. Serves as a Subject Matter Expert on the diagnostic group to which they are assigned.
  • Interdisciplinary Conferences . Creates clinically appropriate synopses and patient treatment objectives when needed for their respective interdisciplinary conferences. Provides all required data to Cancer Conference Coordinator within prescribed time frame. Serves as a facilitator of dialogue about each patient presented at diagnosis-specific Tumor Board. Documents treatment plan/care path decisions in patient?s EMR. Ensures that there is continuity of communication between a patient's various care providers (both within and outside the UNCHCS community) and that there is complete and accurate communication between care providers and patients.
  • Case Management. Reviews patient charts to identify needs, provides guidance in scheduling necessary diagnostic tests, and obtains the results of these tests. Interviews patients regarding their current health practices, identifies health problems, determines the degree of compliance and problems complying with the prescribed treatment plan, and documents this information in the patient's chart. Analyzes data from patient interviews, chart reviews, and physical assessments to determine patient's nursing care needs including direct care or referral to other departments. Appropriately triages telephone calls from the patient with problems or complaints. Maintains written and oral communication with statewide home health agencies, referring appropriate cancer patient to these agencies for emotional support, reinforcement of wound care instructions, and/or nutritional and physical assessment. Additional duties related to coordinating radiotherapy, surgery, chemotherapy, and new patient appointments with individual teams and the specialty.
  • Genetic Screening. Stays current with NCCN guidelines for genetic testing for assigned diagnostic group, alerting providers to potential needs and opportunities for scheduling testing or referrals. Educates patient and family regarding molecular/genetic testing and refers to Certified Genetic Counselor and/or High Risk Program as appropriate.
  • Patient Advocacy and Support. Takes initiative in ensuring that needed patient services are available at the Cancer Center or by referral. Works with health system and community leaders to develop programs and resources as needed.
  • Programmatic Leadership . Provides programmatic and thought leadership regarding patients in assigned diagnostic specialty to all care management units (i.e. Scheduling and Registration, Clinical Research, Infusion Center, Radiation Oncology, Oncology Care Team, Hem/Onc Clinic). This duty would include assisting non-clinical staff understand better how various providers and treatments interact to facilitate efficient care. The Navigator may additionally provide some volunteer, resident and fellow teaching, as appropriate.
  • Care Path Development . Works with Cancer Center leadership, physicians, and clinical partners to develop and refine a clinical pathway that ensures complete and evidence-based care for patients in assigned diagnostic group. Champions compliance with the approved pathway by all providers, utilizing process improvement skills to refine the path when opportunities for improvement are identified. Educates new providers and their office staff on the pathway. Transitions patients to Survivorship Care Path as defined in Diagnosis Specific Care Path.
  • Navigator Continuing Education. In order to be effective, a Clinical Navigator must remain current in their field. To this end, each Navigator should avail themselves of continuing education opportunities. Further, the Navigator must maintain all required certifications. In addition, Navigators should, when appropriate, participate in grand rounds, cancer conferences, and other clinical learning opportunities.
