The COPD Navigator is a professional whose clinical expertise guides patients, families and their caregivers to informed decision making, collaborating with a multidisciplinary team to allow for timely COPD screening, diagnosis, treatment and increased supportive care across the continuum. The COPD navigator will enhance patient access and experience through multidisciplinary team collaboration, acting as a single point of contact, providing for more comprehensive, well-coordinated and personalized care. The navigator assumes responsibility and accountability for coordination of care for a defined patient population by pulmonary specialty to achieve optimal patient outcomes benefiting the patients, their physician
Experience Minimum Required
Five (5) years in nursing experience

Specific training for navigation or coordination of care for the COP patients Knowledge of community resources

Licensure and/or Certifications Required
Registered Nurse in the state of New Hampshire ARC or AHA BLS
