Job Summary
Responsible for providing direction, oversight and management of both ER and community based programs, located in Memorial Hermann's Emergency Departments and the greater Houston region, that strive to connect populations with the services and resources they need to better their lives. The Director partners with hospital business offices and clinical teams and Community Partners throughout Harris and surrounding counties to develop and enhance the ER Navigation and OneBridge to develop and meet measurable outcome goals.

The position oversees the administrative functions of these programs, including budget development, program development, staffing and community partnering. This position will provide support and guidance to staff in identifying and connecting uninsured and underinsured populations with community health and social service resources and benefits. This position will require the incumbent to have extensive knowledge and understanding of issues specific to access of healthcare by uninsured and underinsured populations. Minimum Qualifications

Education: Bachelor's degree in Nursing, Social Work, Public Health, Business or Health Administration. Master's degree preferred

Licensures/Certifications: (None)

Experience / Knowledge / Skills:
  • Eight (8) years of progressively responsible experience working in a community or healthcare setting.
  • At least three (3) years of experience in a leadership position.
  • Advanced computer skills and knowledge of software programs such as Microsoft Office, PowerPoint and Excel.

Principal Accountabilities
  • Provides program development, leadership, management, and administration of all aspects of the ER Navigation and OneBridge Programs to ensure accomplishment to organizational mission and objectives.
  • Develops and implements strategic and high impact community health initiatives and projects to advance the goals of each program. Designs, monitors, and reports on outcomes to leadership, including senior leadership and collaborators, at all stages of the lifecycle.
  • Collaborates with system leadership, business office, care management, clinical departments, physician offices, community leaders, and partner organizations to promote effective teamwork and problem solving to ensure effective utilization.
  • Facilitates growth and develops new initiatives to support the ER Navigation and OneBridge Programs consistent with CHW's learning competencies and in response to the dynamic nature of health care and community environment. Benchmark best practices, network, provide quality management, and other activities as needed.
  • Responsible for the development and ongoing oversight of community engagement, quality improvement and care coordination initiatives; uses data and evidence-based program models to make decisions, plan and implement performance improvement strategies, and drive outreach initiatives.
  • Identifies and develops collaborative relationships, creatively sourcing resources directly through partnerships with others who share Memorial Hermann's Community Outreach goals.
  • Responsible for leading "ER Navigation Managers" who incorporate leadership principles and vision in performing activities concerning community engagement, quality improvement, and care coordination.
  • Responsible for budget management and ensures ER Navigator coverage in the partnering Memorial Hermann Emergency Rooms and the ER Navigation call center.
  • Develop, manage, and maintain relationships with local, regional, state, and national organizations and institutions. Produce and monitor metrics. Strategize and implement program changes as metric results dictate.
  • Generate and write papers, grants and presentations communicating the effectiveness of community programs.
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
