Lead Care Coordinator

The Lead Care Coordinator provides outreach and care coordination services to program enrollees and works collaboratively with BC HP program leadership on best practices, training needs, and development of stakeholder relationships. The Lead Care Coordinator provides rapid outreach to newly referred and monthly outreach to program members, initial and annual assessments and person-centered care plans and other documentation as identified by program requirements. Close collaboration with program leadership is an important factor in the development and implementation of work processes designed with focus on key quality metrics. The Lead works with the team to identify and connect staff with the most appropriate resources for successful completion of their daily tasks, including trainings, job aids, and other resources as needed. With the help of program managers and administrative leaders this position continues to foster close internal and external relationships with collateral providers and referral sources, including but not limited to the CBHC, ACOs/MCOs, DMH and health care centers. Through the course of their work, the Lead Coordinator will identify and share evidence based practices for engagement and service delivery with the team and CP as appropriate.
Essential duties and responsibilities include but are not limited to:
- Works with Riverside BH CP Program Director to utilize various data exchange platforms to ingest and manage BH CP roster.
- Supports Riverside BH CP Program Director in communicating with ACO partners and the Department of Mental Health regarding assignment process and data integrity.
- Closely communicates with program leadership regarding member assignment and other critical information to support the engagement process.
- Provides rapid outreach to newly assigned members. Completing intakes and informational meetings with members who request more information.
- Collects and verifies member contact and provider data for all new referrals.
- Updates member Care Team and utilization data for new and existing assignments
- Completes all clinical and administrative documentation, including progress notes, Assessments, Care Plans, incident reports, etc., in a complete, accurate and timely manner and in compliance with agency and relevant state protocols.
- Supports the onboarding of new staff by providing direct training opportunities, including but not limited to delivering onboarding trainings, allowing staff to shadow them in the delivery of job responsibilities, and shadowing new staff as they perform these job responsibilities to ensure the successful adaption of required job skills.
- Provides routine review of completed outreach activity notes for new staff and communicate necessary corrections to staff and program manager as needed.
- Identifies themes for program improvements, communicate those needs to program management, and facilitate team discussions related to these areas as appropriate.

Required Skills
- Valid driver’s license and regular access to a reliable vehicle required.
- Performs duties in an ethical way that carefully meets the contractual expectations within the program.
Required Experience
- Bachelors Degree in Human Services related field and one (2) year experience working in a health-related setting or, Associate Degree in Human Services related field and one (2) year experience working with youth or, High School Diploma or GED and minimum of two (3) years of experience working with adults living with chronic mental and physical health conditions required.
