Position Summary
Under direct supervision, the Patient Navigator assists with identifying, tracking and resolving health quality gaps in an effort to improve quality and comprehensiveness of patient care service provided and to alert the care management team of opportunities using health plan and EPIC electronic health record information
Job Responsibilities
- Schedules patient appointments. Utilizes tools within the practice management system to expedite the appointment scheduling process.
- Locates patients assigned to the care management team, completes and updates information on gaps in care and links patients to care by making follow up appointments and referrals to services.
- Maintains internal databases, spreadsheets, lists and files related to Care Management and gaps in care.
- Assesses barriers to care and arranges for resolution of these challenges.
- Delivers direct and concrete services to patients to ensure retention in primary and specialty care.
- Identifies patients in need of intensive case management and notifies clinical case manager and supervisor.
- Makes appointment reminder calls in an effort to increase compliance with medical appointments and routine screenings.
- Documents progress in the electronic medical record system and other information systems.
Education
- High School Diploma
Experience
Approximately 2 years of experience in healthcare setting interfacing with patients, identifying needs and resolving access problems
Knowledge, Skills and Abilities
- Experience using clinical or practice management information management systems.
