Job Description:
Summary: This position provides therapeutic management for patient care, supervises technicians, aides and volunteers, and assists in implementing new programs to ensure positive patient outcomes.
Duties/Responsibilities:
- Implements Plan of Care for patients
- Performs complete, accurate, appropriate and timely evaluations on assigned patients according to department policy.
- Plans and implements treatment programs for patients with their input, establishing functional short and long-term goals.
- Administers treatment program using professionally accepted methodology demonstrating knowledge and skills appropriate to the patient's age and status.
- Performs re-assessments on patients in a timely manner, as dictated by WI practice act and DOE requirements.
- Initiate discharge planning for all patients with their involvement, and when appropriate, the family input.
- Performs any physical duty for patients in a safe manner, utilizing appropriate staff when necessary.
- Demonstrates appropriate and safe operation of therapeutic equipment used in patient treatments.
- Attends all meetings/care conferences, which are patient oriented.
- Records patient treatment and patient responses each day.
- Documents objective data pertinent to patient goals.
- Completes daily progress notes and discharge summaries in a timely manner and according to department standards.
- Completes progress notes for MDs and ensures they are received prior to follow-up appointments.
- Appropriately prioritizes patient needs for treatment and exercise.
- Communicates with members of healthcare team.
- Recommends to the Director any educational resource material which may be of value to the department.
- Consults with customers prior to decision making, as well as with other departments and their directors before making procedural changes that may affect them.
- Accurately completes all Medicare-related paperwork in a timely manner.
- Submits accurate daily patient charges.
- Informs Director when unable to stay current with paperwork.
- Trains and supervises student in assessment and treatment techniques expected at the PT level.
- Establishes clear goals and objectives for the clinical affiliation with the student within the first week.
- Assigns appropriate patients to the student to meet established goals and objectives.
- Constructively and objectively critiques and evaluates student on an ongoing basis.
- Completes formal evaluations according to the student's academic affiliation's criteria.
- Communicates with Director any problems, progress and status of the student.
- Participates in QI, patient scheduling, reception of patients, maintenance of equipment and supply closet by purchasing as needed.
- Performs screenings, presents education workshops and performs consultation services.
- Provides adequate supervision to PTA's and ancillary staff in patient care related services.
- Co-signs all notes by ancillary staff and discharge summaries done by PTAs.
Requirements/Qualifications: Qualified candidates will hold a Bachelor’s degree in Physical Therapy with a Masters or Doctorate preferred and possess a valid WI licensure as a Physical Therapist (or have the ability to obtain one). Minimum of 1 year directly related experience is preferred but willing to consider new graduates.
