Job description

We are looking for a qualified registered nurse to promote and restore patients’ health. You will work with a variety of patients and you will conduct nursing assessments to identify the patient's level of care for Medicaid benefit approval.

Responsibilities

• Identify patients’ care requirements, focus on their needs and capture those needs on the assessment.
• Conduct timely documentation and communicate any challenges in a timely fashion.
• Sign off on Level of Care forms and return them timely to the client’s file.
• Provide education to the client and family members of programs available through the Medicaid Elderly and Disabled Waiver Program.
• Follow care regulations and standards
• Work within and cooperate with a multidisciplinary team

Skills

• Proven nursing experience, preferably in Long Term Care
• Ability to use a variety of e-documentation systems with training
• Problem solving skills and ability to multi-task
• Compassionate with good communication skills
• Excellent teamwork skills
• BS or diploma in nursing

Experience

• Must be licensed to practice nursing within the state of Georgia (Compact license is acceptable)
• MDS Experience strongly preferred
• Experience in long term care required

This is an hybrid schedule position with some flexibility of work in the office and remote. Meeting deadlines and completing documentation timely is a MUST, non-negotiable.

Must be available to attend in person meetings every other week, the position is otherwise remote.
