Job Roles
- Supervise assigned staff to include hiring, training, coaching ad ongoing support, conducting corrective action, and providing performance assessments.
- Build positive relationships, inspire others to perform at a high level, prepare career development plans and retain an accountable workforce.
- Conduct regular staff training to ensure objectives, expectations, and standards are being met.
- In collaboration department leadership, develop strategies to increase utilization of specific services.
- Conduct analyses in supporting the planning, development, implementation, and evaluation of services in accordance with contractual and departmental requirements and guidelines.
- Ensure adherence to all relevant policies, procedures, and contractual obligations related to Social Services operations.
- Complete all required program documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
- Ensure staff conduct initial and on-going assessment of client's health and/or support service needs.
- Collaborate with multidisciplinary care team that support a wide array of outreach, health care, case management, and patient engagement functions in supporting and connecting individuals to medical and social services.
- Provide leadership in developing and maintaining positive relationships with appropriate resettlement agencies and other community and social service organizations.
- Ensure guidelines related to program Transportation protocols are being met or exceeded through supervising driver(s) and coordinating transportation procedures with external stakeholders, including resettlement agencies.
- Assess and improve program effectiveness by ensuring cultural and linguistic alignment of service delivery.
- As subject matter expert, provide analysis and recommendations related to current event resettlement efforts for refugees, asylees, and humanitarian parolees.
- Ensure all communications developed meet health literacy standards.
- Provide oversight to the support care team to ensure appropriate coordination of care and delivery of all needed services is being met.
- Provide case management and focus related to patients needing additional services or urgent needs.
- Provide analysis and input related to overall departmental workflow procedures and service levels.
- Actively participate in organizational short and long-term planning and implementation activities.
- Collect data and provide analytical impact for assigned programs through preparation of necessary reports, and improvement plans as necessary.
- Perform other duties as assigned.
Education/Certifications/Licenses/Registrations
- Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field required.
- Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
- Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.
Experience/Specialized skills (including Language)
- 3 - 5 years of work experience providing human services to high risk, medically underserved, or relevant community health populations required.
- 1 year of lead experience or supervisory experience preferred.
- Ability to exercise discretion and maintain confidential information.
- Demonstrated ability to maintain good interpersonal interactions with clients and co-workers as a member of a multi-disciplinary community health team that services a diverse multi-cultural population.
- Excellent interpersonal, customer service, written, and verbal communication skills to include the ability to communicate effectively with patients and family members who speak English as a second language.
- Experience dealing with the assigned population of focus, lived experience preferred.
- Bilingual in appropriate languages; Pashto, Dari, Farsi, or any combination of languages of focus required.
- Ability to provide support, structure, empathy, sound judgment, insight into human behavior and family
- Demonstrated ability to be culturally sensitive, respect diversity and follow trauma-informed care standards.
- Good organizational skills, attention to detail, time-management skills, and strong motivation to meet deadlines and achieve goals.
- Ability to work well in both a team-based environment and independently.
- Knowledge of supervisory practices and techniques, including performance management and employee development.
- Strong analytical and problem solving skills.
- Knowledge of the community resources, health and social services systems in San Diego County, and a willingness to work within these settings when appropriate.
- Basic skills in computer applications and software to include MS Office and Electronic Medical Records to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appointments, etc.).
